Adult & Child Case Management

Local counties contract with HDC to provide case management services to adults and children with serious mental health issues. Case managers work closely with individuals and families to help them access needed supports and services to maximize independence, mental health, and community integration.

Adult Rehabilitative Mental Health Services (ARMHS)

ARMHS provides rehabilitation services to individuals so they can gain skills to live independently in their community. Services are provided to people who have a mental illness, and difficulty in their ability to live independently and meet the medical necessity criteria for service. Participants must be a recipient of or eligible for Minnesota’s Medical Assistance (MA) or PMAP Health Plan programs.

Assertive Community Treatment (ACT) Program

ACT provides the most intensive community services available. Services are provided to persons with serious mental illness (SMI) or serious and persistent mental illness (SPMI) diagnoses referred by St. Louis County or Carlton County Social Services’ mental health units. Services are available 7 days a week, and include medication management, Psychiatry, case management, substance abuse treatment, crisis intervention, employment support, independent living skills training, and housing support. Services are based on the individual needs of the client and are provided primarily in the individual’s home.

Certified Peer Support

Certified Peer Support Specialists are individuals who have lived through mental health experiences. They are trained and certified by the State of Minnesota to provide person-centered services to others with mental health challenges or substance abuse issues. Peer Support Specialists hold a personal belief in recovery and a genuine interest in their peers’ recovery. Peers share their own recovery stories in ways that inspire and encourage them. They provide a model of recovery for others by personally demonstrating recovery is possible.

How can a Peer Support Specialist help me?

A Peer can empower you in using strengths to create a self-determined path to recovery, identifying and overcoming barriers to participation in community resources, building relationships, and supporting community-based activities such as work, relationships, and physical movement, among other things.

Children’s Therapeutic Services & Support (CTSS)

CTSS is designed to help children under the age of 18 with mental health, emotional, and behavioral issues. This service is based on the unique and changing needs of the child and is designed to improve the child’s functioning within the family and community. CTSS activities take place in the child’s home and within the community.

Community Support Program (CSP)

CSP outreach services are provided to adults with the primary goal of assisting consumers in recovery from mental illness.

Interventions include supportive therapy, medication management, education regarding mental illness, assistance with learning needed skills, finding and maintaining affordable housing, independent living skills, benefits assistance, vocational support, symptom monitoring, outreach, crisis assistance, psycho-social rehabilitation, and general case management. Program services are primarily provided on an outreach basis with services typically occurring in the home or community.

Early Childhood Mental Health (ECMH)

The social and emotional development of infants, toddlers, and preschoolers is important, and so are the relationships they have with their caregivers. ECMH services for these children (ages 0-5 years old) and their families focus on enhancing the connection between young children and their caregivers.

Parent-Child Interaction Therapy (PCIT)

PCIT is a great opportunity for families with young children (ages 2-7 years old) whose behaviors are difficult to manage. Families participate in weekly sessions that provide training for parents on how to manage their children’s behavior while improving their relationships with their children.

PCIT has been successful with oppositional/defiant problems, aggressive behaviors, Attention Deficit Hyper-Activity Disorder (ADHD), severe temper tantrums, the impact of abuse/neglect, and exposure to alcohol/drugs before birth. PCIT requires a 12–14-week commitment to a 1-hour-long session per week.

Employment Connections Program

Employment Connection provides high-quality vocational rehabilitation services to clients. These services are designed to enhance the quality of their lives by obtaining and maintaining meaningful employment. We believe “work is therapy,” and all persons have the capacity to be productive members of their community. Employment Connection is specially designed to help adults with mental illness (MI), serious mental illness (SMI), and serious and persistent mental illness (SPMI).

First Episodes Psychosis (FEP)

FEP uses NAVIGATE, a team-based treatment focused on resiliency and recovery. When enrolled, you and your family meet with a mental health professional to answer questions and discuss a treatment plan based on your needs.

HDC offers five FEP services:

  1. Support in learning skills, coping with stress, and identifying your strengths
  2. Education and care for you and your support team
  3. Help with staying in or returning to work and school
  4. Assistance in obtaining community resources
  5. Counseling on medications to reduce symptoms and prevent relapses

Harmony Center

The Harmony Center is located on the lower level of the Damiano Center at 206 W 4th St in Duluth, MN. The Harmony Center is staffed by Certified Peer Support Specialists and provides a safe environment to socialize, explore new hobbies or skills, play a musical instrument, do arts and crafts, play games, or just hang out and relax and have a cup of coffee.  Harmony Center is open Monday, Tuesday, Wednesday, and Friday from 8:30 AM-3:30 PM.

Homeless Program

We believe homelessness is not a choice and, given the opportunity for safe, affordable housing, everyone prefers a permanent place to live. HDC’s Homeless Program serves individuals in Duluth who are homeless, have a mental illness, and may have health problems and substance abuse issues. We provide community outreach to locate people experiencing homelessness and assist them in obtaining and keeping permanent housing. We also provide case management, employment assistance, transportation, benefits assistance, and crisis intervention.

Medication Assisted Treatment (MAT)

MAT is an evidence-based practice proven to reduce the need for inpatient detoxification services, improve patient survival, and sustain long-term recovery. MAT also decreases illicit opiate use and criminal activity among people with substance use disorders, increases people’s ability to gain and maintain employment, and improves birth outcomes for pregnant women with substance use disorders. MAT medications are available for opioid, alcohol, amphetamine, and tobacco use disorders.

Outreach Centers

HDC outreach centers offer persons with mental health disabilities the opportunity to be part of a social club, make friends, and learn new skills. Outreach centers use a consumer empowerment model, integrating consumer-led activities, socialization, community outings, games, music, and support groups with the more traditional staff leadership. While these centers are not treatment programs, they work closely with staff from other HDC CSP services to assist consumers with vocational, housing, and psychotherapy needs.

Perinatal Program

Brightwater Health provides compassionate, comprehensive support for individuals and families through every stage of the perinatal journey.

Our approach centers on compassionate care, evidence-based practices, and an understanding that every perinatal experience is unique.

We provide support for those trying to conceive, navigating pregnancy, adjusting to postpartum changes, grieving a pregnancy or infant loss, or settling into early parenting. Recognizing that this period can bring profound emotional shifts, physical changes and complex decisions, our team ensures that no one has to navigate it alone.

With Brightwater Health, individuals and families can expect a safe, inclusive place to share their experiences, receive expert care, and build a healthy foundation for themselves and their growing families.

T-ACT

T-ACT is a program designed for young adults, ages 18-25 years old, who have serious mental health issues and are homeless, at risk, or otherwise disenfranchised. An assertive outreach approach assists clients with life skills, social skills, housing, jobs, medication management, and other basic needs. The program is collaboratively staffed by St. Louis County Social Services, HDC, and Life House.
